A primary care visit is a comprehensive opportunity to discuss health concerns, receive preventive care, and build a trusted relationship with your provider.

    Confirmation of basic information, insurance details and any change in your medical history or medications.
  2. Comprehensive Health Review
    Record vitals, review of your full medical history including past illnesses, surgeries, medications, allergies, family history, and any recent health or life changes to secure a complete picture of your health.
  3. Consultation and Examination
    Review current symptoms and concerns, address preventive care needs such as screening and vaccinations, review any chronic conditions and treatment plans, discus lifestyle factors such as diet, exercise and stress. A thorough physical exam may include listening to heart and lungs, checking abdomen, skin and reflexes as well as examining ears, eyes and throat.
  4. Personalized Care Planning and Coordination of Referrals and Follow-Ups
    Creation and review of a tailored plan that may include blood work, screening, immunizations, medication adjustments, lifestyle recommendations and referrals to specialist.
